ABSTRACT

Objective@#The study investigated cognitive performance and brain function between treatment-resistant depression (TRD) and non- TRD patients to find potential neurobiological markers associated with refractoriness in depression patients. @*Methods@#Fourteen TRD patients, 26 non-TRD patients and 23 healthy controls (HC) were included in the present study. The neural function of prefrontal cortex (PFC) and cognitive performance among the three group were examined using near-infrared spectroscopy (NIRS) during verbal fluency task (VFT). @*Results@#Both TRD and non-TRD groups exhibited significantly worse VFT performance and lower activation of oxygenated hemoglobin (oxy-Hb) changes in the bilateral dorsolateral PFC (DLPFC) compared to the HC group. Within the TRD and non-TRD groups, VFT performance was no significant difference, but activation of oxy-Hb changes in dorsomedial PFC (DMPFC) in TRD patients was significantly lower than non-TRD patients. In addition, activation of oxy-Hb changes in right DLPFC were negatively correlated with the severity of depressive symptoms in depression patients. @*Conclusion@#Both TRD patients and non-TRD patients exhibited lower oxy-Hb activation in DLPFC. TRD patients exhibit lower oxy- Hb activation in DMPFC than non-TRD patients. fNIRS maybe a useful tool for predict depressive patients with or without treatment resistant.

ABSTRACT

Objective:To study the behavior of seeking medical care for people with disabilities under the background of Health Poverty Alleviation. Methods:Descriptive statistics method was used to analyze the behavior of seeking medical care for people with disabilities based on the data of the National Health Poverty Alleviation Dynamic Management System. Results:The Three Batch action plan mainly focuses on contracted services for chronic diseases; the number of visits for people with disabilities gradually increases, the proportion of visits within the county increases, and the proportion of hospitalizations decreases. The hospitals visited are mainly secondary hospitals, township health hospitals and community health service centers, and the medical expenditure is relatively high. These phenomena are different among disability conditions. Conclusion:The behavior of people with disabilities was remarkably improved after Health Poverty Alleviation. In the future, we should consolidate the achievements of Health Poverty Alleviation, increase the prevention of chronic diseases for people with disabilities, and further improve the accessibility of medical services and the level of medical security for people with disabilities.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical significance of BRAF-V600E mutation in children with Langerhans cell histiocytosis (LCH).</p><p><b>METHODS</b>Real-time fluorescence quantitative PCR was used to detect BRAF-V600E mutation in paraffin-embedded tissue samples from 26 children with LCH. A retrospective analysis was performed for the association of BRAF-V600E mutation with clinical features and prognosis of children with LCH.</p><p><b>RESULTS</b>Of the 26 children, 25 received standard chemotherapy, with a 2-year overall survival (OS) rate of 100% and a 2-year event-free survival (EFS) rate of 88%. Of the 26 pathological samples, 18 (70%) came from bone tissue, and the positive rate of BRAF-V600E mutation reached 50% (13/26). The positive rate of BRAF-V600E gene mutation was not associated with age, sex, affected organ, clinical classification, early treatment response, recurrence, and 2-year OS and EFS rates of the children with LCH (P>0.05), but it was associated with clinical grouping of LCH (P<0.05).</p><p><b>CONCLUSIONS</b>Children with LCH tend to have a high OS rate and a high incidence rate of BRAF-V600E mutation. BRAF-V600E mutation is associated with clinical grouping of LCH.</p>

ABSTRACT

A novel polydimethylsiloxane ( PDMS)-poly-L-lysine ( PLL) microfluidic chip, comprised of 24 reaction channels with 2. 5 μL volume of each reaction channel only, was proposed for quantitative analysis of methamphetamine ( MET) based on time-resolved immunoassay techniques. The chip utilized the adsorption characteristics of PDMS and PLL towards proteins to immobilize MET complete antigens ( MET-BSA) on the surface of reaction channel. Then the competition reaction could happen between MET-Ag in the sample solution and MET-BSA on the inner surfaces of the reaction channels with MET-Ab in the reagent. The surface of latex microspheres was labeled by lanthanide, which could emit red fluorescence under the exposure of ultraviolet ( UV ) . Based on the principle of competitive immunoassay, the more MET-Ag, the less the fluorescence intensity in the reaction channel. The detection results of this chip were acquired using UV-irradiation fluorescence imaging method. With this method, 24 samples could be detected and analyzed simultaneously on a chip by just taking the fluorescence image of the chip. The method allows the detection of MET antigens ranging from 100 ng/mL to 3000 ng/mL, with less sample consumption and high-throughput. This chip is suitable for the police preliminary screening work and has a good application prospects.

ABSTRACT

Aim To observe the effect of thioesterase superfamily member 4 ( THEM4 ) expression on extra-cellular matrix ( ECM ) accumulation in the kidney of diabetic mice .Methods For in vivo vector delivery experiment , male CD1 mice were randomly divided in-to four groups: normal control mice ( Control group ) , diabetic mice ( DM group ) , diabetic mice receiving pYr-ads-4-THEM4 vector ( DM+THEM4 vector ) and diabetic mice receiving pYr-adshuttle-4 vector ( DM+V vector ) .pYr-ads-4-THEM4 vector or pYr-adshuttle-4 vector ( 1 mg · kg -1 ) were mixed with TransIT-EE Hydrodynamic Delivery Solution from Mirus Co .and injected into tail vein once a week for four weeks after STZ injection.Four weeks later, mice were sacrificed and Western blot , immunohistochemistry and real-time PCR were used to detect the expression of phospho-Akt(Ser 473), THEM4, TGF-β1, α-SMA, Col Ⅲ, FN proteins and THEM4 mRNA in kidneys respectively . Results THEM4 decreased in kidney of diabetes mel-litus accompanied with increased phospho-Akt ( Ser 473), TGF-β1, α-SMA and ECM.The delivery of pYr-ads-4-THEM4 vector increased THEM4 expression and decreased phospho-Akt (Ser 473), TGF-β1, α-SMA and ECM deposit in kidneys of diabetic mice . Conclusion The up-regulation of THEM4 may pre-vent ECM deposit by inhibiting the phosphorylation of Akt and down-regulating the expression of TGF-β1 andα-SMA in kidneys of diabetic mice .

ABSTRACT

<p><b>OBJECTIVE</b>To study WT1 gene expression in children with acute myeloid leukemia (AML) and its possible correlations to clinical outcomes.</p><p><b>METHODS</b>Bone marrow samples were collected from 45 children with AML (excluding acute promyelocytic leukemia, AML-M3) at different time points of AML treatment and follow-up. WT1 gene expression levels in bone marrow mononuclear cells were assayed by real-time fluorescence quantitative PCR. The correlation between WT1 expression and prognosis was retrospectively analyzed.</p><p><b>RESULTS</b>The WT1 expression level in AML children with bone marrow blast cell percentage of >60% was significantly higher than in those with bone marrow blast cell percentage of ≤ 60% (p<0.05). The lower WT1 expression level was documented in children with AML-M2 compared with in children with other non-M2 subtypes (p<0.05). WT1 expression level in patients in complete remission was significantly lower than that in patients at diagnosis or relapse (p<0.01). The 2-year disease-free survival (DFS) in patients with higher WT1 expression was significantly lower than in those with lower WT1 expression at the end of induction chemotherapy (p<0.05). The 2-year overall survival (OS) and DFS in patients with ≥1 log WT1 reduction range were significantly higher than those with <1 log reduction of WT1 expression level at the end of induction chemotherapy (p<0.05). WT1 expression levels tended to rise 2-3 months prior to bone marrow relapse.</p><p><b>CONCLUSIONS</b>WT1 expression level is closely correlated prognosis in children with AML. Dynamic monitoring of WT1 expression level is of great clinical importance in terms of individualized management, prognosis evaluation and relapse prediction.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish a standard method for digital evaluation of breast symmetry with 3D scanning technique.</p><p><b>METHODS</b>From January 2009 to July 2010, 167 patients received 3D scanning before breast augmentation. The coordinate system was established and the 3D reconstructed breast models were analyzed by software. The discrepancy of nipple level, the distance between nipple to midline, inferior mammary fold location, breast width, breast projection, breast volume and anterior chest wall projection were measured.</p><p><b>RESULTS</b>The mean discrepancy of nipple level, the distance between nipple to midline, IMF level, breast width, breast projection and anterior chest wall projection were (4. 8 +/- 3.9) mm, (4.5 +/- 3.4) mm, (4.6 +/- 3.7) mm, (4.8 +/- 2.9) mm, (5.4 +/- 3.9) mm and (4.8 +/- 3.3) mm, respectively. The mean difference of breast volume was (51 +/- 44) ml. The incidence of significant asymmetry was 73% (122/167)in nipple position, 95% (159/167)in breast shape, 38% (63/167)in anterior chest wall projection.</p><p><b>CONCLUSIONS</b>3D scanning technique can provide an accurate 3D measurement of breast. A thorough and objective evaluation of breast symmetry can be achieved.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish a rapid method for detecting MTHFR gene 677C>T polymorphisms with high-resolution melting curve method (HRM) and pyrosequencing.</p><p><b>METHODS</b>Peripheral blood samples were collected from 155 Down syndrome patients and 182 normal controls from Children's Hospital of Shanghai. The accuracy of three methods including regular HRM, internal control HRM and artificial heterozygosity HRM was compared. Meanwhile, allele frequencies in 10, 30 and 50 mixed samples were measured with pyrosequencing, and the results were compared with that of HRM.</p><p><b>RESULTS</b>Heterozygosity of 677C>T polymorphism could be distinguished by various HRM methods. However, homozygotes CC and TT were only identifiable by internal control HRM and artificial heterozygosity HRM. The accuracy of pyrosequencing for allele frequency has improved with increased sample number. When the number of mixed samples has exceeded 30, the difference between pyrosequencing results and actual values became less than 4%. TT genotype was more frequent in Down syndrome patients than controls (25.2% vs. 14.3%). No significant difference was found in T allele frequency between the two groups (44.9% vs. 40.1%).</p><p><b>CONCLUSION</b>Respectively, internal control HRM and pyrosequencing may be ideal methods for determination of genotypic and allelic frequencies.</p>

ABSTRACT

Objective To observe the effects of iodine excess on thyroid morphology,the expression of thyroid peroxidase and sodium iodide symporter mRNA and to explore their mechanisms.Methods One-month SD rats were divided into three groups:control iodine(CI),high iodine Ⅰ(HI Ⅰ)and high iodineⅡ(HI Ⅱ)and were fed with water containing iodine in different concentrations by adding K103(5,5000,10 000μg/L)respectively.Rats were sacrificed after being fed for six months.The morphology of thyroid was investigated under light microscopy and electron microscopy,the serum thyroid hormones and ratio of TPO/β-actin and NIS/β-actin were measured by radio-immunoassay and RT-PCR method.Results The major changes were increased follicles with colloid accumulation in HI groups.The levels of serum thyroid hormones TT3 and TT4 were decreased gradually from CI[(75.68±13.99,1.45±0.49)nmol/L]to HI Ⅰ[(73.82±16.48,1.34±0.31)nmol/L]and HIⅡ groups[(70.65±11.43,1.15±0.39)nmol/L],but there were no significant differences among three groups(F=O.371,l.163,P>0.05).The TPO and NIS mRNA expressions in HI Ⅰ(1.28±0.10,0.56±O.17)and HI Ⅱ(1.14±0.04,0.39±0.06)were significantly lower(F=30.863,62.62.675,P<O.05)than those of control group(1.39±0.08,0.71±0.13).Conclusions Chronic iodine excess leads to definite histological changes in rat thyroid,and inhibits the expressions of TPO and NIS mRNA as well as thyroid hormone synthesis,which in turn acts as a protective mechanism against iodine excess.

ABSTRACT

<p><b>OBJECTIVE</b>To study the treatment of primary malignant duodenal tumor.</p><p><b>METHOD</b>The data of 54 cases of primary malignant duodenal tumor during 1993 approximately 2003 were analyzed retrospectively.</p><p><b>RESULTS</b>Clinical manifestations were jaundice, abdominalgia, obstruction of digest tract and bleeding. Correct diagnosis rates of image examination were endoscopic retrograde cholangiopancreatography 92.8%, air barium double radiography 70.8%, gastroscopy 50.0%, CT 21.9%, MRI 21.4%. Tumor location was 1 in duodenal bulb, 45 in descending portion, 3 in horizontal part and none in ascending portion. 48 malignant tumors were operated, 31 pancreaticoduodenectomy, 1 pancreaticoduodenectomy and partial resection of superior mesenteric vein, 6 radical segmental duodenal resection, 1 palliative segmental duodenal resection, 3 duodenal wedge resection, 5 bypass operation (gastrojejunostomy and/or cholangiojejunostomy), 1 jejunostomy. Adjuvant chemotherapy was given in 13 cases. The survival rates were 5-year 45.4%, 3-year 45.4%, 1-year 63.2%. Median survival months were 24, 10, 38 and 16 respectively for radical operation group, palliative operation group, with postoperative adjuvant therapy group and without postoperative adjuvant therapy group. No significant survival time was found between radical operation group and palliative operation group, adjuvant therapy group and without postoperative adjuvant therapy group, pancreaticoduodenectomy group and radical segmental duodenal resection group in statistics. Among lymphyaden metastasis, tumor size, tumor depth, tumor thrombi, pathologic type and operative methods, only tumor thrombi had prognostic significance in multivariate analysis.</p><p><b>CONCLUSIONS</b>Pancreaticoduodenectomy and radical segmental duodenal resection should be selected for primary malignant duodenal tumor. Bypass operation can prolong survival and improve life-quality. Postoperative adjuvant treatment is advocated.</p>
